2. ขั้นตอนการขอต่ออายุใบรับรองจริยธรรมการวิจัย

  1. ดาวน์โหลดและกรอกบันทึกข้อความ บันทึกขอต่ออายุใบรับรองจริยธรรมการวิจัย บันทึกข้อขอลงนามต่ออายุใบรับรองจริยธรรมการวิจัย และ AF 01-11 แบบรายงานความก้าวหน้า พร้อมแนบเอกสารที่เกี่ยวข้อง
  2. นำเอกสารทั้งหมดมายื่น ที่สำนักงานคณะกรรมการจริยธรรมการวิจัย
  3. เมื่อเอกสารครบถ้วนและการพิจารณาเรียบร้อยทางสำนักงานจะติดต่อท่านชำระค่าธรรมเนียม AF 10-05 แบบฟอร์มแจ้งการชำระค่าธรรมเนียม และรับใบรับรองฉบับใหม่
    1. โครงการที่ได้รับทุนจาก มหาวิทยาลัยพายัพ: 500 บาท
    2. โครงการที่ได้รับทุนจาก หน่วยงานภายนอก: 1,000 บาท 

หมายเหตุ: รายการเอกสารทั้งหมดอยู่ในบันทึกข้อความ กรุณาตรวจสอบและแนบให้ครบถ้วนก่อนส่ง

2. Procedure for Renewal of Research Ethics Approval Certificate

  1. Download and complete the memorandum forms Memorandum Renewal of Research Ethics Approval, Memorandum Request Signature Renewal Research Ethics Approval, AF 01-11 Progress Report Form and attach the required documents
  2. Submit all documents in person to the Research Ethics Committee Office
  3. Once the documents are complete and the review process is finalized, the office will contact you to proceed with fee payment AF 10-05 Payment Notification Form and collect the renewed certificate
    1. Projects funded by Payap University: 500 THB
    2. Projects funded by external organizations: 1,000 THB

Note: The complete list of required documents is provided in the memorandum form. Please ensure that all documents are properly checked and attached prior to submission.
